We understand that visiting an orthopedic office can be overwhelming, especially if you're unsure about what to expect. This page is designed to provide you with information you need to prepare for your appointment, and get the most out of your experience with Martin Orthopedic and Wellness.
Once you make your initial appointment with Dr. Jabari Martin, you will receive an email with a link to complete the required intake forms. Please complete all forms prior to your first visit.
There is a $100 no show fee for all missed appointments. If you are unable to keep your appointment, please call our office to reschedule.
We accept a number of health insurance plans for our services. Please click here for the full list. If you do not see your insurance provider, kindly give us a call at (301) 381-2750 to discuss your options.
To schedule a Virtual Visit, please click on Schedule An Appointment and in the message box, be sure to specify “Virtual Visit.” Dr. Martin will call you directly at the appointment time you selected.

Payment is required before medical record information is released.
You will be charged:
- Base charge (to cover supplies and labor): $22.88
- + Per page fee: $0.83 per page; Electronic Releases: $0.62 per page (Maximum charge of $81.63 for the first 100 pages)
- + Postage
